E-Rezept for Patients: How to Use Your Digital Prescription in Germany (2026)
For most patients, this works seamlessly. You leave the doctor's office, walk to your pharmacy, hand over your health card, and collect your medication — just as before, except there's no pink paper form involved. But the system has more options than most patients realise, and knowing them can save you time, a trip to the pharmacy, and occasional frustration when things don't work the way you expect.
This guide explains every way to use your E-Rezept, which method suits which situation, and what to do when something goes wrong.
What Is the E-Rezept? (The Short Version)
When your doctor prescribes a medication, they create a digital prescription that is stored on a central secure system called the E-Rezept-Fachdienst, run by gematik (Germany's national digital health agency). Your health card (eGK) acts as the key to access it — or you can use a smartphone app.
- Nothing is stored on your health card itself. The card simply unlocks access to your prescription on the central system.
- E-Rezepte are valid for 28 days for GKV reimbursement. After 28 days, you can technically still redeem the prescription but would pay the full price yourself (Selbstzahler). Unredeemed prescriptions are automatically deleted after approx. 92 days.
- The pink paper prescription form is gone. You won't receive one unless there is a system outage or you specifically request a paper printout (Papierausdruck).
The Five Ways to Redeem Your E-Rezept
You have more choices than just "go to the pharmacy with your health card". Here is every method available in 2026, when to use each one, and what you need:
| Method | What You Need | Best For | Notes |
|---|---|---|---|
| eGK (health card) at the pharmacy counter | Your health card — no app, no PIN needed | Everyone — the default method | Simply hand over your eGK at the counter. The pharmacist retrieves your prescription. No smartphone required. |
| Pharmacy app (your local pharmacy's app) | Smartphone + your pharmacy's app installed | Patients using a local pharmacy with a digital app | Forward your E-Rezept directly to your pharmacy before you arrive. Choose Click & Collect or home delivery. |
| gematik E-Rezept App | NFC-capable eGK + smartphone + PIN from insurer | Patients who want full digital control of all prescriptions | Manage all prescriptions in one place, send to Click & Collect or delivery. Requires PIN setup. |
| CardLink (via pharmacy or insurer app) | NFC-capable smartphone + eGK | Online pharmacy ordering without a PIN | Transitional technology ending January 2027. Works today but expect the process to change. |
| Paper printout (Papierausdruck) | Paper with QR code from your doctor | When you don't have your card or app available | The doctor prints a code on paper. Take it to any pharmacy; they scan the code. Valid backup option. |
Method 1: Health Card (eGK) at the Pharmacy Counter
This is the simplest method and works for virtually every patient. You don't need a smartphone, an app, or a PIN. Just bring your health card to the pharmacy counter, hand it over (or hold it to the card reader), and the pharmacist retrieves your prescription from the central system.
- Receive your prescription from the doctor. Your doctor enters it digitally. No paper form needed.
- Go to your pharmacy. Any pharmacy in Germany — you are not restricted to a specific one. Bring your health card.
- Hand your card to the pharmacist. They insert it into the card reader. Your prescription is displayed and dispensed. No PIN required for this step.
- Collect your medication. Pay any applicable Zuzahlung (co-payment). Done.
Method 2: Your Local Pharmacy's App
If your pharmacy has its own app, this is often the most convenient method. You can forward your E-Rezept to your pharmacy before you leave the doctor's office, choose whether to collect it or have it delivered, and receive a notification when it's ready.
- Download your pharmacy's app. Ask your pharmacy if they have one, or check their website.
- Receive your prescription from the doctor.
- Open your pharmacy's app and select 'forward prescription'. Scan your health card or log in.
- Choose your collection method. Click & Collect or home delivery (Botendienst).
- Receive a notification when your order is ready.
Method 3: The gematik E-Rezept App
The official E-Rezept App gives you full visibility of all your current prescriptions in one place and lets you send them to any pharmacy in Germany. You will need an NFC-capable smartphone, an NFC-capable eGK, and a PIN sent by post from your health insurer.
- See all your current E-Rezepte in one place
- Forward prescriptions to any pharmacy in Germany
- Share a prescription with a family member so they can collect it for you
- Select multiple prescriptions and send them together
- Check pharmacy opening hours and available services
What to Do When Things Go Wrong
The E-Rezept system is generally reliable, but problems do occur. Here's how to handle the most common ones:
"The pharmacy can't find my prescription"
This usually means the prescription hasn't been released to the system yet (wait a few hours), the system is temporarily down, or the prescription was issued under a different name or card number. Show the pharmacist your health card and ask them to try again directly.
"My health card doesn't work with the NFC reader"
Older cards (issued before 2023) may not be NFC-capable. This only affects app-based methods. Contact your health insurer and request an updated NFC-capable eGK for free.
"I haven't received my PIN for the E-Rezept App"
If you didn't receive a PIN, contact your health insurer directly and request one. The counter and paper printout methods work without a PIN.
"My prescription has expired"
E-Rezepte are valid for 28 days for GKV reimbursement. After that, you pay full price until ~92 days when it is deleted. If deleted, ask your practice for a reissue.
"The system is down at the pharmacy"
Ask if you can leave your health card details and return later, or request a paper printout from your doctor. Pharmacies are required to handle outages.
E-Rezept and the ePA: What's the Connection?
Since January 15, 2025, all E-Rezepte are automatically recorded in your elektronische Patientenakte (ePA — electronic patient record). Specifically, your prescription and dispensing data is stored in the elektronische Medikationsliste (eML), which became available on April 29, 2025. The eML gives you — and any healthcare provider you authorise — a complete picture of all medications prescribed and dispensed. You can opt out of this automatic recording if you prefer.
